Well, I've been waiting to see if something really good was coming up. Now
I dare to come up with my own proposal. Please have the story with it too:
The German Medical Service (something like a controlling agency similar to
the DMERC's) asked the Director of the German Federal School for Orthopedic
Technology to come up with a document just as Brian asks for. The German
Veterans Administration endorsed the idea and the German Professional
Association (BIV) thaught it a good idea for objectifying the amputee's
functional needs and have a proof for the health authorities.
So I designed it at that time and had hundreds of helpers since then to
improve it. Some of them live on the North American continent but do not
want to be named.
The document has been published recently (in German) by the Springer
Publishers as an annex to a book of Gertude Mensch's and myself. Springer
is one of the world's most renowned publishers in the medical field.
Some people really like the document, some others think it's too much.
For those using it for years now it has been a great help. Most of it is
multiple choice and easily answered anyways.
Please use it or do not use it, as you please, but do not publish it or
copy it under your name. The copyright laws are strict. Please refer to the
source (Wieland Kaphingst and Gertrude Mensch) and to Springer Publishers
if you are asked.
You also may want to print out and keep this letter as a proof of
permission. This is an authorization by the author.
Please do me a favor and do, as others did before:

This is version 14, there is ample room for another few changes. And if
someone loves to write access files of such documents, please give me a hand.
