Dear list,
I'm looking for info regarding orthotic treatment for posterior tibialis
tendonitis. My patient is a 50+ yo man who sustained a foot injury
about 30 years ago. Over time, he developed a medially collapsed foot
with hindfoot valgus, hallux valgus and pain. Pain was located at the
1st MT and the distal lateral malleolus. The foot is flexible although
there is forefoot supination with a neutral hindfoot. I fabricated a
UCBL foot orthosis and achieved good hindfoot control and minimized any
forefoot abduction. He did very well for almost one year. Since then
he has had knee pain(related to patella-femoral problems) and some foot
discomfort. It may be that my thermoplastic has fatigued and I need to
re-do in a thermo-set material; however, I'm wondering what others have
done with such a problem. The PT is stretched not torn, surgury is not
a consideration. He is a very active individual and is unwilling to
limit activities. I was only recently able to convince him that having
more than one pair of shoes until they were full of holes was not a
reasonable approach. Your suggestions will be appreciated. Molly
