Thank you for responding! This survey can be taken and submitted online by:

1. Highlight the entire document and either click forward or reply. The
whole message / survey should now appear, unhighlighted, in a compose mail
type of window.
2. Use your mouse pointer to insert the cursor after each question.
3. Type your response in... the text will become inserted into the survey (if
your email program can change the color of your response's text , that would
be very helpful)
4. Be sure to e-mail it to: <Email Address Redacted> (don't post the answers to
O&P-L)

Many of the questions are yes/no questions. There are some questions,
however, that ask for an explanation, an opinion, or a more detailed answer.
Please, do not skip these! Your opinions are just as vital to this project as
the hard data.

If you prefer, you can always print out this document and return it to me via
mail. Mail to: Residency Survey, 1441 Narrow Lane Parkway, Montgomery, AL
36111. If you have any other questions, please don't hesitate to email or
call (334) 286-9919 during work hours.

While ideally this survey applied to residents only, anyone doing or having
done their 1900 hrs, or residency dirctors, please feel free to look at the
survey as respond to questions that apply to you. Please be sure to tell me
that you're a director or a 1900 hours graduate, though.
