I was fitting a guy today and his symptoms fit the pattern perfectly!
Any suggestions?

In all it doesn't look very promising, still I have another idea to try. My
client is much too young and active to give up easily.
Thanks to all who replied. Further ideas and input would be greatly
appreciated.
