research investigation, the following link should allow you to complete
our survey.

We are Doctor of Physical Therapy students from Touro College in Bay
Shore, New York engaged in an area of inquiry related to individuals with
lower extremity limb loss. We are requesting your participation in our
study regarding prosthetic prescription trends. Our research seeks to
gather qualitative information from prosthetists across the country. This
study is part two of a three part research design, which aims to determine
if what is being taught in accredited Doctor of Physical Therapy programs
throughout the United States aligns with what prosthetists are currently
providing for their clients. We have attached a survey which will ask a few
demographic questions about your facility, and general information on your
preferences for prosthetic socket and componentry prescription. The data
collected in this survey will be deidentified to maintain your anonymity.
Your clinical experience and input is of great value to us and we hope that
you will take a few minutes of your day to complete this survey. By
completing this survey, you are consenting to participate in this study.
