Hi Everyone,

*We are hoping to complete this study at the end of the month. Please
consider passing along to your patients. We appreciate your help! *

We are looking for individuals with who are 18 years or older with upper
limb loss at the wrist or higher to complete an online survey. Below is the
information you can forward to them. If you would prefer to hand a flyer
out, please let me know and I can send it along to you.

The purpose of this research is to survey individuals with upper limb loss
or absence to collect information on their prosthesis experiences and fall
history. This survey will help us understand the impact of upper limb loss
on falls and identify ways to improve the safety of individuals with upper
limb loss. Any individual eighteen years and older with upper limb loss at
or above the level of the wrist can complete the survey.

Your participation in this research project is completely voluntary. You
may decline altogether or not answer any questions you do not wish to
answer. Your responses will remain confidential and anonymous. There is no
compensation for your time to complete the survey, which will take
approximately 15-20 minutes.
