Question or direction...

I have always understood that for orthotics to be billed for a patient
while in a nursing home or skilled nursing facility that they must be there
a consecutive 100 days without any discharge and readmits to a hospital
etc.

I have a facility debating with me due to a particular patient who has
frequently gone in and out of the hospital multiple times in August but
they had a bed hold on her so she considers it as if they never left and
has been able to bill for them still. Does this matter or since she left
to go to the hospital it must be 100 days since the last time she returned
to the facility??

Could you please point me in the right direction or to the policy article
that states this requirement.
