Thanks for everybody for the quick responses. question was if the notes such as patient is ambulatory with the weakness or deformity of the foot and ankle…” can be on DWO and not office visit notes.

Everybody replied that:
All notes MUST be on MD’s office visit note. these type of wording won’t be acceptable if they are in the DWO.
